SCREENING INFORMATION
***Please note, you must bring a valid (non-expired) photo ID and your Social Security Card with you to your screening visit. If your ID is expired, you will not be seen. Valid forms of ID include: drivers license, passport, military ID and State issued ID card.
Arrive for your screening appointment early enough to completely read and fill out your paperwork before your scheduled appointment time. We recommend you arrive a minimum of 15 to 30 minutes early.
CONCERNING MEDICAL RECORDS
All diabetic subjects and subjects with diagnosed conditions - Medical records are needed before you can participate or dose for a study. Please make every effort to obtain these records and bring them to your screening appointment. If we are unable to obtain these records, you will be dropped from the study. Please keep copies of your records. You will need them EACH time you screen for a different study. In the future, you will not be able to screen without having these documents at your screening appointment.
Non-childbearing Females - Medical records confirming that you are surgically sterile (have had a hysterectomy, tubal ligation, etc) are needed before you can participate or dose for a study. Please see the above statement for more details.
Concerning Walk-ins - We cannot guarantee that you will be seen. Patients with appointments will be seen first. Walk-ins will be seen based on the number of no shows, that day's schedule and staff availability. You maybe turned away even after waiting several hours.
When calling 210-225-5437, (our recruitment line) select option 1 OR 2 and leave a message clearly stating your name and telephone number. Calls are downloaded several times a day and a recruiter will call you back as soon as possible.
