The future of clinical trials: Why patient-centric outcome measures matter

What are outcome measures?

Outcome measures are the methods used to assess the effectiveness of a treatment. Patient-centred outcomes:

Clinical Outcome Assessments (COAs): Tools that assess how a patient feels, functions, or survives. These include patient-reported outcomes (PROs), clinician-reported outcomes (ClinROs), observer-reported outcomes (ObsROs), and performance outcomes (PerfOs).
Digital Health Technologies (DHTs): Wearable devices and mobile applications that can passively and continuously collect real-world data, offering continuous insights into a patient’s daily functioning and health status.

By combining actively collected COAs with passively gathered digital biomarkers, clinical researchers can gain a more comprehensive understanding of how a treatment impacts patients’ lives outside of the clinic.

Why outcome measures matter for drug development

The shift toward patient-centricity is not simply about improving trial design—it ensures that new therapies truly meet patients' needs. Guidance from regulatory agencies such as the FDA and EMA also encourage sponsors to demonstrate meaningful patient outcomes. For example, the FDA’s four-part guidance series on patient-focused drug development (PFDD) underscores the importance of incorporating patient experience data from early-stage research through post-marketing studies. Similarly, the EMA has issued recommendations emphasizing the integration of patient perspectives in drug development.

This regulatory evolution presents both opportunities and challenges for sponsors. While designing trials around patient priorities can accelerate regulatory approvals and enhance treatment adoption, researchers must carefully determine which outcome measures best capture what truly matters to patients.

By integrating both physiological assessments and quality-of-life measures, sponsors can create more patient-centred trials from phase I through post-marketing studies. This comprehensive approach helps ensure that:
